Population Health Data Analyst
Job Description
SUMMARY:
Under direction of the Population Health Director, the Health Data Analyst is responsible
for reviewing payer lists and researching patient charts to validate new HCC codes.
Researching will entail reviewing outpatient records for referral reports; calling
referring physician offices; reviewing discharge summaries and logging into hospital
systems to collect information on the patient's condition. The HDA will be responsible
for communicating closely with the physicians, and will enter appropriate HCCs into
the patient's chart. The HDA will work closely with the Population Health team to
ensure that HCC codes added to the charts are appropriate, and that risk adjustment
goals are being met.
ESSENTIAL DUTIES AND RESPONSIBILITIES:

- Responsible for accessing monthly HCC reports from various payers for our 5 primary care clinics, and possibly Pediatrics.
- Researching “dropped” HCC codes and communicating with providers as appropriate.
- Responsible for logging into various websites, including the hospital system, in order to extract appropriate diagnosis codes, medications, and other tests, as appropriate.
- Adding appropriate HCC codes to the patient’s problem list in outpatient EMR.
- Responsible for cleaning up problem lists, as appropriate.
- Attending monthly payer meetings to update payers on issues related to risk adjustment.
- Work with the Population Health team to ensure that HCC codes are being assessed and billed.
- Maintain chart integrity by resulting back preventive labs and exams as needed.
- Maintain organized documentation of completed and incomplete work.
- Ability to maintain focus for long periods of time and work in a sedentary position.
- Work with the Population Health team on other duties as assigned.
EDUCATION AND/OR EXPERIENCE:
Must possess an LPN license or at minimum a Medical Assisting certification. High proficiency in Microsoft Products, particularly in Excel and Access in required. Experience in EPIC Electronic Health Record preferred.
