Youth with Disabilities or on an IEP

Helping you navigate resources

The transition process during and after school for youth with disabilities can be incredibly complex, and you deserve age-appropriate care. With this transition comes the responsibility to manage one’s health and health care as independently as possible, and this will look different for each individual according their specific needs. Ask your doctor what paths would be best for you and your family as you begin to navigate this path. While it may seem challenging, there are many resources to help guide you and your parent(s) in this process. Here are some resource by different areas of need.
